Tax Form - Annual Summary Report (ASR)

Your Annual Summary Report (ASR) is a breakdown of all your SJPOA deductions for the year (your dues, donations, and life insurance premiums). You will need this to complete your taxes and we mail these out at the end of every January.

  • Please let us know if you move so that we can update your file and ensure your ASR reaches you.

  • Now is a good time to review your beneficiaries on file.

  • ASRs and your beneficiaries are not posted on our website.
